Most actors are lucky to have one defining character in their career, but Sylvester Stallone has both Rocky and Rambo. Rocky became an instant feelgood classic in 1976 and Stallone has revisited the character in various sequels and spinoffs, "retiring" with Creed II. For First Blood, numerous actors like Dustin Hoffman and Steve McQueen had passed on playing Rambo before Stallone signed up.

Rambo is another underdog story in a sense, with the franchise jumping between the gritty tone of the first movie to the cartoonish action of Rambo III, which ends with the title soldier driving a tank into a helicopter. Stallone's original concept for Rambo V was based on the sci-fi action novel Hunter, where a Rambo-esque tracker hunts down a genetically engineered monster. Rambo: Last Blood underwhelmed financially and received mostly lukewarm reviews, with even the character's creator David Morrell stating he was "embarrassed" to have his name on it. The movie ends on an ambiguous note for John Rambo, following a finale where he lures an army to his ranch and systemically slaughters them all. A badly wounded Rambo than sits on his front porch and the end credits play over clips from previous entries, charting his journey from young warrior to a weathered old man. Rambo: Last Blood's Blu-ray comes with the featurette “Drawing Last Blood: Multipart Production Diary,” which is split into five chapters. The final part "Nothing Is Over" takes about the character's arc, but Stallone himself is quick to note the final scene is open-ended by design and Rambo isn't going to die. The last shot of the movie sees Rambo saddle up on his horse and ride into the distance, which apes the ending of classic Western Shane. This image could be viewed as the character riding off to die or even his dying thoughts, but that's not the case.

While Rambo: Last Blood was billed as the final movie and Stallone himself claims he can't imagine coming up with a more emotional final story, the star has a history of returning for sequels he's claimed he'd never make too. Even during the promotion of Last Blood, he said he'd be open to another movie and even hinted it could take place on an Indian reservation following the events of the fifth film.
