Of the many threats and dangers in Pathologic 2, the sentient Plague is one of the main adversaries that Artemy Burakh, also known as the Haruspex, must deal with to survive and save the Town. The Plague only has one acknowledged form yet goes by several names, including Sand Plague, Sand Filth, Sand Ulcers, Sand Fever, Sand Pest, Sand Dirt, and Sandy. Having a strong Immunity will prevent Infection in this survival horror, but there's no guarantee that the player will assuredly avoid contracting the deadly Plague of Pathologic 2.

The Plague is frightening in Pathologic 2, yet dying is cheaper. Still, it will only impose itself upon those who succumb. Fortunately, there are four ways to cure oneself of the Plague, one of which can only be conducted much later during a playthrough, specifically on Day 9. Regardless, players with a burning desire to survive should administer one of the available remedies for Sand Pest in Pathologic 2.

The three primary options for curing the Plague in Pathologic 2 are Panacea, Shmowder, and fire. The fourth method is connected with the in-game achievement, "Big Heart," and becomes available on Day 9 during the Abattoir quest in this survival horror. To be cured, speak with Zurkhen’s Big Chamber after jumping into the pit leading to the "earth's body." After "touching the Heart of the Town," the protagonist will no longer be afflicted with Sand Plague.

Panacea is the first method of curing the Plague in Pathologic 2. Unlike Shmowder, Panacea contains no drawbacks and completely recovers the one who consumes it, including NPCs. The Haruspex can brew Panacea by combining Tinctures with Living Blood. However, Living Blood is a rare resource, resulting in only a maximum of seven vials of Panacea being available throughout one playthrough.

The next means of Sand Pest cure is Shmowder, an antidote that functions similarly to Panacea but will reduce the user's to 10% of its existing value. Fortunately, it is not likely one will die from consuming Shmowder, yet the player's health will be reduced substantially, depending on their current pool of HP in this survival horror. Unlike Panacea, Shmowder can be obtained more frequently through the world, but the player must actively search for it, or learn about a source's whereabouts during specific events.

Finally, the cheapest, easiest, yet riskiest method of curing the Plague in Pathologic 2 is getting caught on fire. One quick way to intentionally set oneself ablaze is by provoking an NPC armed with molotovs. However, it is highly advised to have at least 50% HP or more, as it's possible to die from burning after being cured.
