It's revealed in the fourquel that the original Elder died off-screen, which meant that a new Elder was recruited in his place. And there are still some huge gaps in the lore that will need to be ironed out in the future. However, the fourquel makes a significant leap in giving more details about the Elder. Most importantly, it explains that if one Elder is killed, another person will simply take his predecessor's place. And that means, as John Wick shot dead Georgiou's version of the elder, then yet another person will take his place. Though it doesn't make any sense why anybody would want the role, as becoming the Elder seems like the mark of death.

Regardless of whether or not they want the job, another Elder will take Georgiou's place. And as the latest two films in the franchise have depicted the Elder as an evil individual and the only one in the assassin's world who isn't governed by ridiculous rules, they must be the overarching villain of wherever the series goes next. The franchise has proven that it's great at worldbuilding more than anything, and the two confirmed John Wick spin-offs can continue that winning formula. Ballerina could reveal who the new Elder is, and as The Continental spin-off series is set in the 70s, audiences might be introduced to a much older Elder to.

Not only that, but Wick was wandering the desert for days trying to find the Elder in John Wick: Chapter 3 - Parabellum, but in John Wick: Chapter 4, it was barely an inconvenience. That could be a sign that Georgiou's Elder wasn't the right person for the role. Either way, these issues have to be cleared up in Ballerina.
