Tammy had a medical emergency towards the end of 1000-Lb Sisters season 3. She was rushed to the hospital, and had a trach tube implanted when she left. The TLC reality star had barely been in the food addiction rehab for a day before the emergency. She reportedly got the trach tube inserted sometime before Thanksgiving 2021, meaning she had a trach tube for a little over a year before removal. She said that her reliance on the device was temporary.

Tammy's trach tube was capped in September 2022, a hint that removal was coming. For the 1000-Lb Sisters star, this phase was the last before the tube was removed. Redditors on a thread started by @megazoot commented that it will be impossible for her to get the surgery while she has the tube. Luckily, since she got the trach removed, Tammy underwent surgery, and the results are staggering.
