Thanatos is one of the first characters players will encounter, but to romance Thanatos in Hades Zagreus will need to acquire some specific items. Players aren't able to romance all the characters in Hades, and there are actually only two to choose from. Luckily, these are by far two of the best options in the game, with one of them being Thanatos the God of Death.
The first thing that players will need to do in order to romance Thanatos in Hades is get six bottles of Nectar and gift them to Thanatos. This can be fairly difficult because he is one of the more difficult characters in Hades to track down. Basically, players will have to get lucky while out on runs and find him out in the Underworld and hope he reappears inside the House of Hades.

Once players encounter Thanatos and gift him Nectar six times, they will be able to begin his romance in Hades.
Hades: Thanatos Romance Guide
Once all six Nectars have been given, it is still up to the player to romance Hades' Thanatos or continue as friends. This is made slightly more complicated by the fact that, unlike the majority of the characters in Hades, Thanatos' favor isn't explicitly stated for the player, but it is pretty easy to complete as long as they just play the game how they have been so far.
After giving Thanatos six Nectars and beginning another run in Hades, players will eventually come across Thanatos out in the Underworld again. This may not happen on the immediate next playthough, but as long as all the Nectars have been given players can rest assured Thanatos will appear again eventually. When this happens he will challenge Zagreus as usual, but this time he wonders if Zagreus is strong enough to escape.

After beating him this time Thanatos realizes how strong Zagreus really is, and at this point players can begin gifting him Ambrosia. To romance Thanatos in Hades players must continue gifting him Ambrosia until they completely level up his affinity level. Once it reaches the maximum point, a short cutscene will play out in Zagreus' bedroom inside the House of Hades. This is the moment where players are given the choice of starting a romance with Thanatos in Hades or staying friends.
