After weight loss surgery, 90 Day Fiancé star Angela Deem wished to get a facelift, but Dr. David Saddat refused to operate on her, earning the praise of TLC viewers. Having undergone a gastric sleeve and breast reduction plastic surgery on 90 Day Fiancé: Happily Ever After?, Angela wished to look younger in addition to losing 100 pounds. But despite the risks that came with it, Angela puffed away on cigarettes, even smoking immediately before and after the procedure, against her doctor’s wish. Episode five saw Angela visit Dr. Saddat for chin liposuction and a facelift, with Michael being shocked at the $25,000 bill. But in episode six, the surgeon refused to fix Angela’s “turkey gobble,” saying smoking and the surgery don’t mix.

When 90 Day Fiancé fans first noticed Angela looking thinner by the day on her Instagram, many had wondered if she was still smoking. While they also noticed Angela’s reduced breast size, some joked over where Michael’s wife will store her cigarettes with her chest purse missing. But serious conversations about Angela’s smokes and Cokes took place thanks to her antics on 90 Day Fiancé: Happily Ever After?, where she admitted at the age of 55 to having smoked for 43 years. The doctors warned Angela to stop and explained the risks, but stubborn Angela smoked even after having a sip of green juice, in an act of having been “enough healthy for the day” and treating herself.

But episode six of Happily Ever After started off with Angela revealing that she was asked not to go ahead with her facelift by the 90 Day Fiancé star's patient coordinator Natasha Fett. And Angela still visited Dr. Saddat to impress him with her “Southern charm,” as the Los Angeles surgeon outright refused before adding she would have to put smoking off for four weeks before the facelift. Dr. Saddat explained that they could look at postponing if not canceling it, and spoke of the blood vessels that help to heal the skin being choked due to the smoking. The scene ended with Angela being made to sign a “smoking waiver,” which received unanimous cheers for Dr. Saddat from 90 Day Fiancé fans on Reddit.

We are aiming for perfection,” Dr. Saddat told the 90 Day Fiancé star, adding that her skin could die from it, and said she couldn’t remain his patient. “It was so refreshing to see a doctor actually putting their foot down with Angela,” user Catladydiva wrote, as they called out Angela’s earlier doctors for even going ahead with the other surgeries. "The doctor even went further and was requiring a nicotine test. To make sure she wouldn't sneak cigarettes in during the 4 weeks," they wrote. The TLC viewer also mentioned how Angela wants to get “all these physical changes done but is not willing to make the necessary internal and health changes.

It’s interesting how 90 Day Fiancé legend Angela mentioned she’s had a medical career all her life when she declined to sign the contract handed by Dr. Saddat. But she also disregarded post-op protocols for her darling cancer stick. Eventually, in the episode, a disappointed Angela did admit that Dr. Saddat is a good doctor while saying that she’s met her match. But it's unknown if Angela really will quit smoking and clean up the mess she herself has made in the end.
