Tammy hasn't had the easiest run in 1000-lb Sisters. In season 1, Tammy was enthusiastic about finally prioritizing her health as she and Amy Slaton-Halterman committed to qualifying for bariatric surgery. Regrettably, Tammy failed to follow through on her doctor's instructions and hadn't lost enough weight to get the green light for weight loss surgery. In 1000-lb Sisters seasons 2 and 3, Tammy continued to fall short of her weight loss goals and even put on more pounds by spending time with feeders and enablers. By the end of 1000-lb Sisters season 3, Tammy had pushed herself to the limit and has been in recovery since. Although the TV personality is teasing her return, 1000-lb Sisters fans think Tammy Slaton might not be in season 4.

Although she's currently in a rehabilitation center, Tammy has maintained an active social media presence which is based on fans' desire for more content from the Slaton sisters. Tammy first confessed that viewers would have to wait for season 4 to see the weight loss progress she's made in rehab, but when asked when the next season would air, Tammy confessed filming hasn't even started yet. While some fans now believe Tammy's an unreliable source, others think Tammy's condition means she won't appear in 1000-lb Sisters season 4 at all. One Reddit user theorized, "[TLC is] very likely filming [season 4] right now."
Although Tammy shared that 1000-lb Sisters season 4 has not yet begun filming, some viewers believe that the upcoming chapter of the Slaton family's story is in production but without Tammy along for the ride. Fans believe Tammy could be missing from 1000-lb Sisters season 4 because of the difficult stage of her life she's stuck in.
