Tammy Slaton, one of the famous sisters from the 1000 lb Sisters series, may not return to season 4 of the show as there is a possibility she may become a ward of the state. The show first aired in 2020, and it followed the lives of sisters Amy and Tammy Slaton, who both wanted to begin weight-loss journeys for personal reasons. For Amy, she wanted to start a family with her husband, Michael Halterman and needed to lose weight so that she would not have a complicated pregnancy. On the other hand, Tammy's journey was necessary as her weight had caused her to become nearly bedridden. Over the seasons, fans watched as Amy worked hard to make her dream a reality while Tammy did not seem to be taking the journey seriously. There were times when Tammy would give fans hope that she was doing better with her journey, but most times, they were left feeling disappointed, much like Tammy's family, who has been taking care of her for many years.

At the end of season 3, Tammy was admitted to the hospital after suffering from carbon dioxide poisoning. Tammy had ended up in a coma, and once she was revived, a tracheotomy was placed in her neck to ensure that sufficient air got to her lungs. Since this issue, Tammy has been in a weight-loss facility where she keeps fans up to date through her social media accounts, mainly Tik Tok and Instagram. Fans shared a proud moment when Tammy took to Tik Tok to share that she had lost weight. This proud moment did not last long as it was revealed that Tammy's video reveal was heavily filtered and another image was released which showed Tammy in a very similar state as she was when she entered the facility. Of course, Tammy was once again being looked at as someone who did not care much about working on herself.

As fans await announcements on whether the Slaton family will be returning to TLC for the next season, there is uncertainty as to whether Tammy may be able to return to the show. According to All About the Tea, Tammy's siblings, Amy, Chris, and Amanda, have been discussing what their next step would need to be to ensure Tammy gets proper care, and Chris believes Tammy needs to become a ward of the state. If this happens, the siblings would no longer be responsible for caring for Tammy, and her likelihood of appearing on the show would be decreased. Though Tammy likely wouldn't be able to get weekly updates, Chris is making the right decision that this would be the best for Tammy as she always knowingly indulges in things that harm her. Additionally, the responsibility of caring for Tammy can sometimes be burdensome for the remaining siblings.

The show may lose its familiar feeling as Amy is also contemplating returning to the show. At the end of season 2, Amy gave birth to her son, Gage Halterman, and her focus shifted from helping take care of Tammy to her family. Now that Gage is a part of the family, Amy is finding it difficult to film. Whenever Amy is set to film, she usually has to leave Gage with his grandparents, which causes her to feel like a bad mother and experience mom guilt. In addition, Amy recently revealed in a YouTube video that she was pregnant with another baby boy. Therefore, filming would become more taxing for the soon-to-be mother of two. Amy told The Sun that she loves the crew of the show and would be willing to film if the focus of the show shifted more towards Chris and Tammy. Also, Amy would prefer to film only twice a week so that she still has time to focus on her family. The show also places unnecessary stress on the family's relationships as they encounter several spats about different disagreements.

With all these likely changes, there is no guarantee that there will be  1000 lb Sisters season 4. When fans had inquired from Tammy about whether there would be a new season, she mentioned that filming had not yet started and it was going to take a while before the show aired on the network. When Amy was asked, she said that she was unaware as TLC had not yet reached out to the family to discuss the future of the show. Fans believe that the show would not survive without Tammy, and if both sisters are gone, the show is set for failure. While TLC is still figuring out the logistics for the possibility of a new season, if Tammy becomes a ward of the state, it will ultimately change everything.
